We have some of these built out to audit parts of the stack, setup in recurring tickets with the worksheets linked.
Is this referring to the “Will show on ticket PDF if tag is included”? I’ll say that within our workflow I don’t believe we have ever used a ticket PDF, its all email communication and then invoice.
If there is a “Tag” that can be thrown into the body of a ticket to link the results of the worksheet as a ticket comment, there should be a button at the bottom of worksheet for something like “Copy tag” so it can be easily used. (or alternatively, “post worksheet results as ticket comment”)

We are using worksheets for new hire setups, terminations, workstation decommissioning and some typical scheduled “auditing of client services”.
I will mention that we only use them as “checklists”. Entering data is a shortcoming of this feature, as we don’t see or find a way to reference the data input into the worksheet fields.
We rather put all of the relevant data into ticket notes, so it can be searched (lol, funny joke) also so that the ticket notes show within the labor line item on customer invoices.
It would be amazing say you “finish” a worksheet, and then all of the lines of the worksheet be converted to a ticket note, that would be SWEET.

I toyed with trying to build out a Quarterly Business Review, since you can share the Worksheet results with the Enduser. Not sure how useful this would be compared to a built-out report, but might also work for an Initial Client Assessment or similar.

I think this is a great idea! or maybe even convert to ticket note AND email worksheet results to End User. Being able to link a Worksheet with time/tracking labor point is also an interesting idea.

If you want the End User to fill out a Custom Form you need to build a Custom Ticket Type and Custom Widget Flow, which I outlined in this community post .
Optionally you can watch (before the Q and A in the 7-11-25 Power Hour Webinar) a setup walkthrough I did that matches with the community post.
Each Custom Widget has its own URL which you add to the System Tray
Incase others are interested in other Agent Contact Form, Worksheet, etc functionality, I describe other aspects to consider below:
For Technician ONLY facing Worksheets, they can be auto-attach to a ticket based of the Custom Field Type of Ticket it is.
You can have different Agent Contact Forms open tickets in specific “Custom Field Ticket Types”
Steps for setup:
- Find the Ticket Custom Field Types page in Admin
- In my example I have a custom field type called “New User / Hire Request”
- Bottom Left you can set the Worksheets to attach to the ticket automatically.
- For the Agent Contact Form, you can have different Contact Forms open up in a Specific Custom Field Ticket Type
